How to Apply for a Veterinary Care Voucher Card

To be eligible to receive a Veterinary Care Voucher Card through One Health Organization, you must register to become a One Health Family Member. This application form is for those who have never been a One Health Family Member. If you are a returning One Health Family Member, visit this page instead.

Only apply if you believe you're eligible. Pick one of these two options to apply.

1. Fill out the online application form below. Remember to hit the "Submit the Application Form" at the bottom to complete your application. 
2. Call the office at (216) 920-3051 extension 511 to leave a voicemail with the spelling of your first and last name, mailing address, and daytime phone number. A paper application will be mailed to you at the address provided.

Completing the application process does not guarantee you'll receive a Voucher Card loaded with funds when needed. Voucher Cards are distributed on a first-come, first-served basis and based on the availability of funds for eligible households. Previous One Health Family Members will take priority over new applicants.
