Not Accepting New Applicants

As of 9/12/2023, we are not accepting new applicants until further notice. We plan to create a new process for people who believe they are eligible and want to be placed on a waiting list to learn when they may apply. We appreciate your patience and understanding.

For Current or Past One Health Family Members

If you are still on the waiting list, you may claim your Voucher Card until 3/31/2024.  

If you received a Voucher Card, we will let you know when you can reapply for more funds.

If you are not on the current waiting list and never received a Voucher Card, we will let you know when you may apply.

Note: Current and past Members will have priority to receive funds over those who are not already part of our Circle of One Health Family Members.

How does the Voucher Card work & what does it cover?

Voucher Cards can pay for most veterinary care goods and services that are medically and surgically recommended by a veterinarian. This only applies to vet care for dogs or cats. Search through our list of Veterinary Partners that accept Voucher Cards (red pawprints; select "Accepts Vouchers" from the dropdown list) as payment and confirm whether they are accepting Voucher Cards as payment. 

$250 should cover veterinary care needs for one healthy dog or cat needing an annual wellness visit that requires a physical exam, some disease tests (e.g. heartworm test for dogs, FIV/FeLV test for cats, and fecal exams), and core vaccinations (annual shots for adult dogs and cats). If you have puppies or kittens needing their first series of vaccinations, more than one pet (count only dogs or cats), or a sick pet, the vet bill(s) may be much higher than $250 in a one-year cycle.

Vouchers may NOT be used for the following items.
  1. Paying off old debt is NOT covered. You must make arrangements with your veterinary clinic to pay off your debt.
  2. Tail docking and ear cropping to meet breed standards are NOT covered.
  3. Goods and services that aren't medically or surgically necessary for your pet's health (or for your health benefit), such as microchipping, collars, leashes, toys, etc. are NOT covered.

Eligibility Requirements

There are three basic requirements.

  1. A dog or cat must live with you. 
  2. You must live in Cuyahoga, Geauga, Lake, Lorain, Medina, Portage, or Summit County, Ohio.
  3. You must have an income at or below 200% of the current federal poverty guidelines.

Acceptable forms to prove financial need include the following.

  1. Medicaid card
  2. SNAP card
  3. WIC card
  4. Benefits letter from the Social Security Administration (SSA) - see the Table below
  5. Benefits letter from Social Security Disability Insurance (SSDI) - see the Table below

You must provide a copy of any one of the documents listed above during the application process.

The Table below lists monthly household income based on the number of people living in your home.

Waiting List

Despite our best efforts to raise money to support the Voucher Program for everyone who asks for help, there may be times when we cannot provide you with a Voucher Card or the funds to cover all your vet care needs.

If we do not have the available funds AND you can wait to get veterinary care, you may be placed on a waiting list. We will contact you once you can claim your Voucher Card (or refill your Voucher Card with funds). It may take six (6) months to collect sufficient funds.
