Renew your Membership

Fill out the form below to renew your Membership starting on 4/1/2024. This is the quickest way to process your application and be first in line to get Voucher funds. The Membership renewal cycle ends on 12/31/2024. Your Membership benefits will end on 3/31/2025. If you are new to the program, visit this page instead.

Otherwise, call the office at (216) 920-3051 extension 511 to leave a voicemail with the spelling of your first and last name, mailing address, and daytime phone number. A paper application will be mailed to you at the address provided.
